The right care and support programs are vital to an individual’s success. At Missouri MENTOR, we work with individuals, families and case workers to select and design programs that provide the right balance of independence and support.
Our partnership approach is focused on personal choice for the person receiving services, as well as those providing care. Our ongoing program assessments ensure that we adapt to changing needs of those we serve.
If you or a member of your family has an intellectual, developmental disability, or if you’re a case manager or social worker seeking alternative or first-time placement for an individual in your community, send us an email or give us a call at our state office: 314-991-7944.
Please note: In the state of Missouri, individuals seeking services for someone must also contact a State of Missouri Service Coordinator with the Department of Mental Health from your home region. The following link will take you to the DMH site, where you’ll find links to regional offices. From there, you can acquire information on how to contact a service coordinator.
